To apply for individual credit please start by clicking on the file below and printing it. Complete section B. If applying for joint credit (cosigner) complete section C as well. Once completed, sign the bottom of page 2, and fax both pages to 978-538-9911 (Attn Alex Dytko).
Corporate Credit Application
To apply for corporate credit please start by clicking on the file below and
printing it. Complete sections B & C. If applying for joint credit (cosigner or
personal guarantor) complete section D as well. If there is no cosigner or
personal guarantor, then be advised that financial statements (most current year
end and prior year end including footnotes) are required from business
applicants.
Federal income tax forms may be submitted in lieu of business
financial statements. The 1040 forms must be supported by appropriate business
related schedules such as Schedule C.
Once completed, sign the bottom of page 2,
